The reason why Mafuyu forgot about Tsukasa (and vice versa) is just some sekai-fuckery and to make it "canon-compliant" since both of them doesn't know each other in canon, that's pretty much it (sweats) but other than about each other they remember everything else really
as for bad ending 3, you do need to pick specific choices... and they are:

thank you for the answers, i got the ending and oh my gosh no my heart...i cant believe he chased after and they both died together, it was so sad... :,) at least there's still the good true ending hehe :3
