Is this the issue with the new Squad Leader icon?
Please verify you don't have an outdated mod that edits mapstyle, such as improved zoom / improved mapstyle. There are other mods out there that also edit these files. Any one of them can cause you problems if it hasn't been updated or you are using an old version. I try to put the foxhole version at the end of mine so you can tell if it's up to date or not.
