When and how can you "toggle" the fetishes? And which fetishes can you even toggle?
They're in the settings I believe, in the menu you see before restarting a loop. From what I remember, you can toggle pegging and possibly lesbianism
From the Orrery, click Options and the toggle list will show pegging, pregnancy, and happy mode.
