You mean the 7z file? Only 2 less known think it is one. It's a false positive. If you are concerned the first 2 are clean: Win3DMaze.7z, Win3DMaze11.7z. You'll mostly miss out on the mod tool, but it can be done manually in the txt files in every version.
You can also check out the source code on GitHub and compile the game if you really want to make sure.
The game uses external txt and png files. I think it might be the reason that some antivirus flagged it.
Another solution is to play it in a virtual machine, the game doesn't need a powerful PC, it can fall back to Directx 9.
