I finally got a perfect run: A full-clear with 13 hearts leftover at the end!
That can only happen if you do not use any health refills until you've looted all treasure chests, picked up the gnome, and killed the Mine King and cleared out all mines.
It took many attempts, and more than a few... educated guesses? You'll need to take some risks to do this, but usually just inferring which squares are most likely to be chests without actually seeing the minotaurs.

After several tries I did it!
In the successful run I killed the slime mage very early and found the crystal orb a bit later which revealed the rat king and one watcher.
I recorded almost the entire run, maybe I upload it somewhere.

There’s actually a little bit more leeway than that: your first health refill needs to be when you have 11 hearts, and you can get to 11 hearts by clearing only the first 6 of the 9 mines.

True. I forgot clicking on one mine before the first healing but I still ended with 13 hearts.
