Regarding points 2-3, I am afraid those would go beyond the scope of this adventure anthology. Details for Lothorian can be found in SOULBLIGHT - A SHADOWDARK SETTING, along with a rough region map - but I encourage you to devise your own Lothorian or, as you suggest, feel free to drop these adventures with some alterations into Cursed Scroll #2!
Separate Player-Facing Maps will be added soon for all adventures.
