It's something you said to moon that triggers her death. When you talk with her in the kitchen or somewhere sitting don't exactly remember where I remember when Kei was sitting with her later in the game before the fight.

It depends when you choose hot to kill her in the battle between Aoife and moon when I play chapter 1 I choose to not kill her you must not be paying attention of reading or just click it so fast and don't bother to read it I suggest you play it again I play it 2 time and I choose to spare her.
