You have to get one character from school to 10 points in intimacy ( the school girls )
any of them and I don't think ( the sister character and the other Dents friend character) will work, just any of the other girls in the school will do
Pretty sure you need to finish the main story first and unlock the cousin (doctor) cmiiw
