Is this one perhaps related to the Japanese „sub-cult-classic” Dice Quest (だいす★くえすと) and/or its spiritual successor Family Dice (ファミリー・ダイス)? I know I really like those back in the day, especially the first 3 in the DQ line and its horror themed spin-offs.

Please do, by all means! Crazy that it's from 1988 / 1989! Long before Greywulf's 3 Dice Dungeon or Ganesha's Four Against Darkness. Way ahead of its time! Well, sorry for the off-topic comment. Looking forward to this one here, though!

I’m afraid nowadays most of them are only available from domestic vintage retailers, secondhand stores and online auctions. They must be pretty expensive, though... The original materials of the IP’s spiritual successor, Family Dice, seem to be still available (in Japanese) on the designer’s site:
