I think they are trying to keep the game realistic, so they haven't added anything like that yet
Google “intersex" and see if it's not realistic.
Maybe their still working on coding it 🤔 after all it's not easy to integrate a feature in every scene, plus reactions of npcs a d events.
They didn't ask about Intersex, I'm aware that that's a thing, they were asking about hermaphrodites, which is not the same thing. If they were Intersex or something and wanted representation, they would have said that.
