I believe Than must have been first cause if you look closely, there's rope about their neck the entire conversation. So MC must be speaking to their ghost since MC is at death's doorstep.
I thought about adding it while working with the writer, and it just felt like a “fun” detail to add for people to notice or speculate that he is indeed a ghost at the start of the game. Good eye op!
