Look at the video closer. When you hit certain food amounts you have to burp and then click on which body part you want to grow. Each level up requires more food next time to level up that body part again.
It was designed to be more of a passive game. For example you are playing a different game while this one is running in the background to collect points over time.
Unfortunately the game wasn't designed with clear instructions because the game would of been to easy.
