Takedown notice for 'GB Neo-Classic'

To whom it may concern at itch.io,
I am writing to submit a formal notification of copyright infringement pursuant to the Digital Millennium Copyright Act (17 U.S.C. § 512).
I am the original creator and copyright holder of the game "GoreBox," including all associated source code, game assets, and intellectual property. It has come to my attention that the following game hosted on your platform is infringing on my copyright:
• Infringing work: GoreBox : Neo Classic
• URL of infringing content: https://gbneoclassic.itch.io/gbneoclassic
My original work can be found here:
https://store.steampowered.com/app/2027330/GoreBox/
The developer of the above game was granted a limited, non-commercial license to use my source code strictly for non-profit purposes. This license explicitly does not permit monetization of any kind. The developer has violated the terms of this license by integrating advertisements into the game to generate revenue. This unauthorized commercial use exceeds the scope of the license granted and constitutes copyright infringement.
I have not authorized, licensed, or otherwise permitted any commercial use of my source code or derivative works based upon it.
I have a good faith belief that the use of my copyrighted material as described above is not authorized by me, my agent, or the law.
I declare under penalty of perjury that the information in this notification is accurate, and that I am the copyright owner of the intellectual property that is allegedly being infringed.
Please remove or disable access to the infringing material as expeditiously as possible.
