Try add something called a death zone, I had this idea for a while for my own mod.  A death zone is an area of the health bar where BF is at risk of dying.  If the song ends with the player in the death zone, they will be immediately killed.  A player must be over the death zone to win, and the percentage of the death zone will increase with the difficulty.
