I'm pretty sure the Large Rat's follower gimmick is on purpose since it's been like that for a while. It's an easy fight if stay at a small Size and don't use followers, since none of them do barely any damage if you don't hit the small fry.
Edit: It's harder now with the AI change since the little rats don't move out of the way as much. Using a knife or staff still works, though.
