Hi! I don't know about second lap without lil' chads but you can take one of those babies for the red achievement:Lil Chad, Cube of Meat, Spike Baby, Tooth n Nail, Lil Loki or Double Baby.

you can do no-chad achievement on a lap. i have done my on lap 4.
also someone on the wiki said in the comments that having the candy heart does not count as using an active item

No-C.H.A.D. can be done on a loop, but it's also harder to do it on a loop.
