I consider the game to be not really "incomplete" without the NSFW patch, so probably more the latter. At the very least, I would say it tends to follow normal narrative logic over hentai logic. But sexuality is an important aspect of the human experience, and I think a game that more openly addresses that can be more "true to life." And there is a decent amount of adult content if that's to your tastes.
