Added an Example Notesheet

Based on feedback, I added a reference page of how I use the dots & callouts design to take notes. Its now also a demo (just a screenshot of a page of my own notes from a game). Obviously you can change how you find it useful, but since its pretty different than anything out there, people should have something to give them some ideas to start.
