If you do not play the 1.5 update, and would like to fill all the journal entries, you must make a new save in order for them to populate. You can go into the UI turn on skip unseen, and skip in game to fast forward to Dinner with Aldric, once you're there you're have all the entries from Chapter 1, and you will not have to do this step again.
