I ran a testing session on the game myself, which i felt was possible as the game design was not mine originally.  This was centered around playability of the game and ensuring that the game ran as intended.  This informed the creation of the questionnaire, which was designed not to be too onerous and therefore lacking in actionable insight.

Although not within the spirit of the exercise which i wasn't able to effectively participate in due to work commitments, I did feel that I was able to develop a high level framework in the questionnaire which would provide valuable feedback to a creator of a game.
