Someone requested an A4 version, and so here is my attempt! If I understand the paper sizes correctly, when this is folded up it will be an A6 size.
I don't have a way to print and test this, but looking at the PDF, it seems correct. If someone decides to print and fold up the A4 version, please let me know if it worked (everything is centered correctly, etc).
The instructions are still in the main document.
