General Fixes and Improvements Update

- Fixed an issue that caused one of the Max Life expansions to be missable if you died between two specific events.
- Replaced one of the locked doors on the first dungeon with a door that opens when you defeat the miniboss. Also removed the corresponding key.
- Changed the red and green levers with a single one that shows both colors.
- Changed several dungeon doors to no longer close when you die or leave the map.
- Changed some dungeon puzzle elements to stay out of your way after you get past them the first time, even if you die or leave the map.
