Minor corrections

Thank you to everyone who has pointed out that there is an error in the map (transposing points G and H). This has now been corrected on both versions, along with an error in one of the lists being out of sync between numbers and text.

Since I've been asked for it, I've also added a map without the points and routes between them, such as might be found/sold or otherwise given as a handout to the adventurers.
