Vergence 0.16b released

Vergence 0.16b is a minor update that provides a few refinements and two new challenges. 

• We were confusingly using “game mode” and “game phase” interchangeably. Changed to consistently use “mode” throughout the text.
• Adjusted Oracle special abilities.
• Added  two new challenges: Grand Tour and Succession.
• Added information on the worlds of the Agorian Penumbra. 
• Added advice on managing game modes to the Running the Game chapter. 
• Added an annotated example of GM decisions in a conflict scene to the Running the Game chapter.
• The handouts now have page references to the game book.
