Some basic fixes, including fixing a rare,  longstanding UI quickslot bug.

• Fixed rare instance of duplicate abilities being added to the quickslot bar.
• Fixed camera not being centered on loading a new zone/save.
• Fixed Shellworm Plate mistakenly giving 4% Phys. Evasion on weapons instead of 5%.
• Gate: Onslaught weapon damage reduced from 80% to 60
