Here's the difficulty menu. It can only be accessed once, at the very beginning of the game.
Gauge meters should make it easier to judge how well you're doing - they show up during patient sessions, the job interview, the tour of the hospital and the conversation with Dr. Sharpe at the end of the demo where he judges how well you've done - this one should be particularly useful in determining where you've failed and what approach you should take when you retry.
There are currently six tutorials placed throughout the demo. They give instructions on basic gameplay mechanics and serve as hints. You should be able to trust them.

Testing the gauge meters meant I had to go through all of the patient sessions on the first week. To my surprise I actually found a bug and some typos. I also received a list of minor issues from a friend and fixed those as well. So in short:

I also realized I've been publishing one devlog a week recently. I wasn't aware of that.
