I do not understand how to get lost in the tower to get this achievement. I have had runs go well over 100 floors in standard settings and in boss rush, so I presume it's not simply losing after 50.
Any help would be awesome, it's my last achievement. Only have to figure out the milking facility puzzle (if it's even clearable) and then determine if Gwen has a ceiling to her CON resist skill check lol. Got her to 200 once and just sighed internally.
This one is pretty easy but I do agree that the achievement need a better description.
-Whenever your hero gets defeated in a tower battle by an enemy that does not have a Bad Ending, he gets 'lost' in the tower.
To unlock the achievement you need to lose to an enemy after the 50th floor
-About the facility, it really depends on which puzzle you're referring to, but keep in mind that if there is no gray square on the minimap, that room is not implemented in-game

By puzzle... is there a way to beat the facility that isn't the bad ending or running away? I never can find a way into the canteen rooms or the sheds. I have gotten to the one room after 1800 but the note there doesnt really make any sense to me.

Nope, the Facility's main purpose is to gather loot and find new scenes, but having a big boss for the player to beat is an awesome idea
That note is just a reference to one of HC2 bosses, Dannie & Debbie. If you keep investigating that room you will find a legendary recipe for an exclusive accessory tho
