Excellent combination of art style, music and sound. Especially during the levels, where it's mostly silent aside from sound effects. The two samurai standing off with the cherry blossoms blowing in the wind would also make a great screensaver or animated desktop background.
I like the simplicity of the gameplay - it is tense, and leaves your fingers twitchy as you go through the levels. However, the margin of error does feel extremely slim on the last few levels.
I think it is a good idea to have the start and the next level key be separate in order to prevent the player accidentally advancing to the next and starting the countdown immediately.
If there were one improvement to be made, it would be to have the buttons be on the same keyboard row. As the required buttons were near each other (and because I was lazy), I ended up positioning my fingers and thumb in a slightly uncomfortable triangle layout so that I only needed to use one hand to play.
A simple and well polished game.
